This is not the same as our Notice of Privacy Practices. Health information created in the course of your care is governed by that notice. This policy covers the website.

Please do not send medical information through this website

Published email addresses are not a secure channel for medical information and are not monitored continuously. Do not use them to describe symptoms, ask a clinical question, request medication, or report an urgent problem. What we collect

Information you give us. This website does not offer a general clinical contact form. If you use a third-party scheduling tool, such as Calendly for a social-services conversation, that provider may collect the information you choose to submit under its own privacy practices. Do not include health, coverage or identification information in that scheduling flow.

Information collected automatically. The website may process limited technical information required to deliver and protect the site, such as browser and device information, pages requested, referring site information, IP address, and the date and time of a request. If you choose analytics, Google Analytics may also process technical and usage information about visits to this site under Google's privacy practices.

Information you do not have to give us. We do not ask for your Social Security number, Medicare number, member ID or financial-account information through this website. If a page appears to ask for any of these, stop and call one of our centers.
