It starts with a call

Call 786-691-1400. A person answers, in English or Spanish. On that call we confirm whether we accept your plan, pick the center closest to you, and give you an appointment.

Before you call, have ready

• Your Medicare card
• Your health plan card, if you have a Medicare Advantage plan
• A photo ID

What happens at your first visit

1. Reception: we confirm your information and coverage.
2. Health history: we review your conditions, medications and history.
3. Visit: your primary care physician examines you and makes a plan with you.
4. Before you leave: we explain the next steps.
